The Perinatal Community Health Work Program is dedicated to supporting birthing people and families through culturally responsive, community-based care. Our trained health workers provide vital connections to services, education, and advocacy throughout the perinatal journey.

The SUCCESS Toolkit

The SUCCESS Toolkit is part of the Safe Reduction of NTSV Cesarean Births Initiative led by the Kentucky Maternal Morbidity & Mortality Task Force. Designed for clinicians and healthcare systems, it offers evidence-based resources to help lower unnecessary cesarean rates—especially for first-time moms carrying a single baby in head-down position at full term.
